By signing this consent form, I HEREBY AGREE to waive any and all rights whether implied, expressed or consented to and willfully elect to receive chemical services having forgone the patch test and/or strand test (positive patch test would indicate a predisposition to allergic reactions such as skin irritation, redness, swelling, sores, burning, or other reactions).
